C7-T1 Transition

Cervicothoracic (C7-T1) Junction Strain

The cervicothoracic junction (C7-T1) is the transition between the mobile cervical neck and the rigid thoracic ribcage. It bears tremendous shear force from forward head posture, causing "coat hanger" tightness, burning at the base of the neck, and tension headaches.

CLINICAL PRESENTATION

Common Symptoms

  • Burning "lump" or persistent tightness at the base of the neck where it meets the shoulders
  • Tight, rope-like bands in the upper trapezius and levator scapulae muscles
  • Tension headaches starting at the base of the skull and radiating behind the eyes
  • Stiffness when turning the head side-to-side (checking blind spots while driving)
CRITICAL CAUTION

Movements to Avoid

  • βœ— Aggressive head rolling or circular neck cranking
  • βœ— Holding mobile phones down in the lap looking straight down for hours
πŸ’‘ CLINICAL PHYSICAL THERAPY PEARL

Perform 10 McKenzie chin tucks every 2 hours. Tucking the chin glides the cervical facet joints posteriorly and immediately removes 20+ lbs of shear strain from the C7-T1 junction.

CLINICAL QUESTIONS & ANSWERS

Frequently Asked Questions about Neck & Upper Back

Q What is the "dowager’s hump" at the base of the neck?
It is a postural fat pad and soft-tissue thickening that forms at the C7-T1 junction to protect the spine against chronic shear forces from forward head posture.
Q Do chin tucks really reduce neck and upper back pain?
Yes. Cervical retraction (chin tucks) activates deep cervical flexors (longus colli and capitis) and decompresses the suboccipital and C7-T1 nerve roots.
