Scapular Release

Rhomboid & Interscapular Pain

Rhomboid and interscapular pain presents as intense, focal burning or stabbing knots between the medial border of the shoulder blade and the thoracic spine (T1-T6). It stems from prolonged postural strain and weak lower trapezius fibers.

CLINICAL PRESENTATION

Common Symptoms

  • Persistent sharp knot or hot burning sensation between the spine and shoulder blade
  • Pain exacerbated by reaching forward with the arm or typing without forearm support
  • Tenderness and palpable muscle spasms along the medial border of the scapula
  • Relief when pressing a lacrosse ball or foam roller against the knot

Movements to Avoid

  • βœ— Aggressive over-stretching of already over-lengthened rhomboids without strengthening lower traps
  • βœ— Lifting heavy weights overhead with rounded shoulders

Rhomboids are often over-lengthened and exhausted from holding up slouched shoulders. Combining gentle rotational stretches (Thread the Needle) with lower trap strengthening (Y-T-W) permanently resolves spasms.

Q Why won’t the knot between my shoulder blades go away?
The rhomboids are frequently "locked long" due to slouched forward shoulders. Stretching them without strengthening the serratus anterior and lower trapezius leaves them perpetually fatigued and in spasm.
Q How do I release an active rhomboid spasm quickly?
